These two codes cover full-mouth scaling in situations that neither a routine cleaning nor scaling and root planing describes. D4346 is for generalised moderate to severe gum inflammation — bleeding, swollen, tender gums throughout the mouth — in a patient who has not lost attachment or bone. D4355 is a full-mouth debridement: the deposits are so heavy that the dentist genuinely cannot examine or chart the mouth until they are removed, so a gross removal is done first and the evaluation follows at a later visit. Both are single full-mouth procedures rather than per-quadrant ones.

When this treatment is used

D4346 fits the patient with widespread gingivitis whose probing depths remain within normal limits — commonly someone who has not seen a dentist in a few years but whose bone is intact. D4355 fits the patient whose teeth are obscured by calculus to the point that diagnosis is impossible.

The code that filled a real gap

Before D4346 existed, a patient with severe generalised gingivitis and no bone loss presented a genuine dilemma. A routine prophylaxis did not describe the work involved — which might be ninety minutes of dense calculus removal — and scaling and root planing was not clinically justified because there was no attachment loss. Offices either under-reported or over-reported. The code resolved that, and it has become important for a specific group: adolescents and young adults with heavy inflammation, orthodontic patients, and people returning to care after a long absence. If a treatment plan proposes four quadrants of scaling and root planing for someone in that category, this code is worth asking about.

What debridement is not

D4355 is explicitly a preliminary procedure to enable evaluation, not a treatment. It cannot be reported on the same day as a comprehensive examination or a periodontal evaluation, because the whole point is that the examination could not be performed until the deposits came off. A definitive plan follows at the next visit, and it will usually involve scaling and root planing. Plans limit it tightly — often once every few years — precisely because it is easy to misuse as a substitute for a cleaning.
