Arch — upper. The palate provides broad support for anything removable, but the sinuses sit directly above the back teeth, which constrains surgical work.
Counted — per arch — upper or lower. A quote for "the whole mouth" is two of these, which is the single most common misunderstanding on estimates in this group.

D6111 — Implant overdenture — lower (full arch): It is the lower-jaw version.
D6114 — Full-arch fixed implant denture, upper: It is a full upper arch of fixed teeth screwed onto implants, a prosthesis the patient cannot take out.
D6115 — Implant/abutment-supported fixed denture, edentulous arch (mandibular): It is the lower-jaw version.
D6116 — Implant/abutment-supported fixed denture, partially edentulous arch (maxillary): It is a fixed implant prosthesis for an upper arch that still has some natural teeth, so it spans part of the arch rather than all of it.
