D0170 — Re-Evaluation (Limited, Problem-Focused)
D0170 is the follow-up look at a problem the office has already examined — checking how healing or a symptom has progressed.
D0170 covers the second look. Someone came in with a swollen gum, a deep restoration that might settle down, a traumatised front tooth, or a post-surgical site, and the dentist wants to see it again in a week or two before deciding anything. That follow-up is not a new exam and it is not a recall exam, so it has its own code.
It is one of the more under-reported codes in dentistry. Plenty of offices do these visits and write them off, partly because coverage is patchy and partly because a short no-treatment appointment feels awkward to bill.
The situations it fits
Typical uses: reviewing a tooth after an indirect pulp cap to see whether the nerve is settling, re-checking a suspicious soft-tissue lesion after two weeks to see whether it has resolved on its own, following up a traumatic injury to a front tooth over several months, or reassessing a periodontal area after non-surgical therapy before committing to surgery.
What it does not cover is routine post-operative care that is already bundled into the surgical fee — removing sutures after an extraction, for example, is normally part of the extraction, not a separate re-evaluation.
How it differs from a check-up
The distinction plans care about is scope. A recall evaluation surveys the whole mouth on a schedule; this code looks at one issue because something prompted it. The documentation that supports it is a chief complaint in the chart — the patient's own words about what is wrong — and a finding that relates to it.
That is also why reporting two exam codes on one date rarely works. From a payer's point of view you cannot simultaneously have had a broad survey and a narrow investigation of the same mouth on the same afternoon.
